These programs provide additional support. As a Lam employee, you and your family members can use Health Advocate, no matter which medical plan you’re enrolled in.

You must be in an Anthem medical plan to access the autism, diabetes, fertility, or medical travel benefits.

Health Advocate

With Health Advocate, you and your family, including your parents and parents-in-law, have a 24-hour health concierge to support you. Turn to Health Advocate for help with health insurance and clinical issues. Health Advocate is available at no cost to you, and you don’t need to be enrolled in a Lam medical plan to use it.

Autism spectrum disorder (ASD) support

Through the Anthem autism spectrum disorder (ASD) program, your family receives support from a dedicated team of clinicians and case managers who provide you with resources tailored to your unique situation.

What you get

  • Clinical review of your child’s applied behavior analysis (ABA) to help ensure your child gets the right care from the right provider at the right time
  • Coordination of care by case managers, who develop a customized care plan for your family, which helps to identify available services, secure care you may be missing, and facilitate collaboration among treatment providers
  • Connection to resources to help build a strong foundation of care and family support

​What you need to know

  • There is no cost for Anthem members to use the ASD program.
  • You don’t need a referral, although your doctor may refer you to the program.

Diabetes support

If you or your covered dependents are living with diabetes, the Transform Diabetes Care program from CVS Caremark helps make life easier, at no cost to you.

  • A connected glucose meter that automatically uploads blood glucose readings and makes them available to your care provider, who can then provide immediate feedback via the monitor’s messaging feature
  • No-cost strips and lancets, shipped right to your door
  • Coaching anytime and anywhere from certified diabetes educators, available 24/7 by phone, text, and mobile app to answer your nutrition and lifestyle questions
  • Extra support at your CVS pharmacy or MinuteClinic

​What you need to know

  • If you fill a prescription for insulin or other diabetes-related supplies, you can expect to receive a packet in your home mail from CVS Caremark, including information about enrolling in the Transform Diabetes Care program
  • You pay nothing to participate in the Transform Diabetes Care program.

Anthem fertility support

If you need help growing your family, Anthem’s fertility support partner, WINFertility, is here to provide guidance, education, and support at no cost to you.

What you get

  • Up to $25,000 (lifetime) for covered infertility treatment and supplies, including diagnostic tests, artificial insemination, in vitro fertilization (IVF), assisted reproductive therapy (GIFT, ZIFT), medications administered in a physician’s office, reconstructive surgery (except for sterilization reversal), supplies, and appliances
  • Up to $15,000 (lifetime) for prescription fertility medications
  • 24/7 access to specially trained nurse care managers who can answer questions, help you find doctors, and listen to your concerns
  • Referrals to local, high-quality reproductive endocrinologists in Anthem’s network
  • Authorizations for outpatient and in-office infertility services
  • An individualized care plan with treatment recommendations based on your risk level and the success rate of options available to you
  • Information about the causes of infertility, different treatments and medications, and finding the right doctor for your needs
  • Help understanding the fertility-preservation process and selecting an in-network provider
  • Guidance about medication usage, storage, and side effects

What you need to know

  • WINFertility benefits are available at no cost to you and your covered dependents who have an infertility diagnosis.
  • To use this service, you should be referred by your Anthem medical provider.

Kaiser fertility support

Kaiser members have access to the following fertility services.

Assisted reproductive technology (ART) services include in vitro fertilization (IVF), gamete intrafallopian transfer (GIFT), and zygote intrafallopian transfer (ZIFT). Kaiser does not offer egg or embryo freezing for long-term storage.

Medical travel reimbursement

If you or a covered dependent must travel to receive medical care you cannot receive near your home, your Anthem plan will reimburse eligible travel expenses.

What you need to know

  • You can be reimbursed up to $5,000 for travel and lodging associated with a covered claim.
  • You must attest that you were not able to obtain the services from an in-network or out-of-network provider near your home.
  • First, file a claim for the covered medical services. Then, complete the Anthem travel claim form and include the applicable medical claim number and all itemized expense receipts.
